Evaluation Process
To be evaluated for Crohn’s disease under the CUP:
- Confirm your diagnosis with your gastroenterologist
- Book a telehealth evaluation with a CURT-registered physician
- Bring documentation: diagnosis records (endoscopy / colonoscopy / imaging reports), list of prior treatments, current medications
- Physician evaluates whether cannabis is appropriate for your case
- If certified, physician enters you in CURT and you can access a TX-licensed dispensary
What to Bring to Your Evaluation
- Medical records confirming the Crohn’s diagnosis (GI notes, endoscopy / colonoscopy / imaging reports, pathology)
- List of medications and biologics tried and their outcomes
- Government-issued Texas ID
- Insurance card (optional — Texas CUP is not covered by insurance, but useful for the physician’s intake)
- Gastroenterologist contact info for care coordination
