Evaluation Process
To be evaluated for terminal illness under the CUP:
- Confirm your diagnosis and prognosis with your treating physician
- Book a telehealth evaluation with a CURT-registered physician
- Bring documentation: diagnosis records, prognosis documentation, list of prior treatments, current medications
- Physician evaluates whether cannabis is appropriate for your case
- If certified, physician enters you in CURT and you can access a TX-licensed dispensary
What to Bring to Your Evaluation
- Medical records confirming the diagnosis and prognosis
- List of medications and treatments tried and their outcomes
- Government-issued Texas ID
- Insurance card (optional — Texas CUP is not covered by insurance, but useful for the physician’s intake)
- Treating physician / palliative team / hospice provider contact info for care coordination
- Caregiver or legal representative documentation if applicable
